Dreamweaver Foundation's mission is to bring joyful experiences and meaningful connection to older adults. At Dreamweaver Foundation, our programs enhance the emotional, social, and spiritual health of older adults, and their loved ones too. Whether making a bucket list item come true, connecting older adults with loved ones, or something as simple as a holiday card wishing them well – we inspire joyful memories despite the effects of life-limiting illnesses. Together, with help and support from community partners, the Dreamweaver Foundation team creates a world where every older adult feels hopeful, cherished, and connected to someone who cares.
